What Bloodwork Should Bodybuilders Get? A Guide to Key Health Markers
A practical, evidence-informed overview of blood tests bodybuilders may encounter, why context matters, and how trends can support better health conversations.
Bodybuilding asks the body to adapt to resistance training, deliberate changes in energy intake, and—in competitive phases—periods of unusual physiological demand. Bloodwork can add objective information to that picture. It cannot, however, diagnose a condition on its own or replace symptoms, history, examination, and professional judgment.
There is no universal “bodybuilding blood test.” The useful set of tests depends on age, sex, health history, symptoms, medications, family history, training phase, and a clinician’s judgment. The categories below are a conversation framework, not a prescription that every athlete needs every marker.
Start with the question, not the longest test list
A useful testing plan begins with purpose. Is the goal routine preventive care, investigating a symptom, establishing a baseline, or following a previously unexpected result? That question changes what information is relevant and when it should be collected.
Pre-test conditions matter too. Recent hard training, hydration, illness, fasting status, supplements, and medications can affect some measurements. Record those conditions and follow the preparation instructions provided by the laboratory or healthcare professional. Standardizing repeat tests can make comparisons more meaningful.
CBC and general blood health
A complete blood count (CBC) reports red blood cells, hemoglobin, hematocrit, white blood cells, and platelets. Together, these measurements give context about oxygen-carrying cells, immune cells, and clot-forming cells.
An out-of-range result is not a diagnosis. Hydration can influence the concentration of components in blood, while altitude, illness, and other factors may also matter. Significant or persistent changes deserve clinical review rather than a bodybuilding-specific assumption.
Cardiovascular markers: lipids and ApoB
A standard lipid panel generally includes total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides. These markers are interpreted alongside blood pressure, smoking status, diabetes, family history, age, and other elements of cardiovascular risk.
Apolipoprotein B (ApoB) provides a related but different view. Each major atherogenic lipoprotein particle carries one ApoB molecule, so the measurement approximates the number of those particles. LDL-C describes cholesterol mass within LDL particles; the two values can be discordant. No single lipid value captures total cardiovascular risk, and athletic performance does not eliminate the need for preventive care.
Metabolic health
Glucose-related markers may include fasting glucose and hemoglobin A1c. A1c estimates average glucose exposure over roughly the previous two to three months, but certain blood conditions can make it less representative. Triglycerides, waist measurements, blood pressure, and clinical history can add context.
A comprehensive metabolic panel (CMP) also reports electrolytes and other measurements affected by fluid balance and metabolism. A clinician can determine whether additional testing is relevant for a particular question.
Liver-related markers
AST and ALT are commonly discussed as liver-associated enzymes, but they must be interpreted in context. AST is also present in skeletal muscle, and strenuous resistance exercise can raise AST and ALT in some people. Bilirubin, alkaline phosphatase, GGT, symptoms, alcohol exposure, medications, supplements, and the pattern over time may all contribute to interpretation.

Kidney-related markers
Creatinine is used in equations that estimate glomerular filtration rate (eGFR). Because creatinine is related to muscle metabolism, unusual muscle mass can affect creatinine-based estimates. Diet, supplements, hydration, recent exercise, and medications may also be relevant.
That does not mean an unexpected creatinine or eGFR result is automatically harmless in a muscular athlete. Urine albumin, repeat measurements, cystatin C, or other evaluation may be considered by a healthcare professional. See Creatinine and eGFR in Muscular Athletes for a closer look.
Thyroid markers
Thyroid-stimulating hormone (TSH) is commonly used as an initial thyroid test; free T4 and other tests may be added depending on the result and clinical question. Energy availability, illness, medication, and other factors can influence thyroid-related physiology.
Fatigue, temperature sensitivity, heart-rate changes, and performance changes are nonspecific. They should not be used to self-diagnose a thyroid condition.
Hormone markers
Hormone testing is most informative when connected to a legitimate clinical question and collected under appropriate conditions. Depending on the person and question, a clinician may consider total testosterone, free testosterone or binding proteins, estradiol, luteinizing hormone, follicle-stimulating hormone, prolactin, or other markers.
Time of day, sleep, acute illness, energy availability, and assay methods can influence results. A single number should not be used to make medication or hormone changes without qualified care.
Inflammation, iron, and PSA
High-sensitivity C-reactive protein (hs-CRP) is a nonspecific inflammation marker and can rise for many reasons, including recent illness or tissue stress. It does not identify a cause by itself.
Ferritin reflects stored iron but can also change with inflammation. Iron studies are best interpreted together rather than treating ferritin as a standalone performance score. Testing may be particularly relevant when symptoms, diet, blood loss, or history raise a question.
Prostate-specific antigen (PSA) is used in prostate-health assessment for appropriate patients. Whether and when to test involves age, risk, symptoms, and shared decision-making. It is not a universal bodybuilding marker.
Why trends can be more useful than snapshots
Reference intervals describe the range observed in a laboratory’s reference population; they are not personalized targets and do not draw a perfect line between health and disease. A result can be inside a range yet still warrant attention in context, or outside it for a temporary reason.
When repeat testing is clinically appropriate, trends collected under reasonably similar conditions can help distinguish a stable personal pattern from a new change. During physiologically demanding phases such as bodybuilding contest prep, noting the phase and collection conditions becomes especially important.
