Can Hard Training Raise AST and ALT? What Athletes Should Know

Strenuous exercise can affect AST and ALT, but elevated results should not automatically be blamed on training. Learn what broader context can show.

AST and ALT are often called “liver enzymes,” so an elevated result can be alarming. That shorthand is incomplete: these enzymes are used in liver evaluation, but AST in particular is also found in skeletal muscle. Strenuous training can affect both measurements in some athletes.

Exercise is one possible piece of context. It is not a diagnosis and should never be the automatic explanation for an abnormal result.

What AST and ALT measure

Aspartate aminotransferase (AST) and alanine aminotransferase (ALT) are enzymes inside cells. ALT is more concentrated in the liver, while AST is present in liver, skeletal muscle, heart, and other tissues. Cell stress or injury can release them into circulation.

That is why the same laboratory pattern can have different explanations. The values must be considered with symptoms, history, alcohol exposure, medications, supplements, infections, metabolic factors, and other laboratory findings.

What hard training can do

Resistance exercise creates controlled skeletal-muscle stress. A small study of healthy men found that an unfamiliar, intensive weightlifting session significantly increased AST and ALT, along with muscle-associated markers such as creatine kinase and myoglobin; AST and ALT remained elevated for at least seven days in some participants.

That finding demonstrates possibility, not certainty. The study was small, responses varied, and an athlete’s real-world result may have a different cause.

Why AST can be especially responsive

Because skeletal muscle contains AST, muscle stress can contribute to serum AST. ALT can also change after intense exercise, though its distribution differs. When creatine kinase rises in parallel after demanding training, that pattern may add muscle context—but it still does not rule out a liver problem.

Timing matters. A blood draw soon after a high-volume or unfamiliar session may capture a different state than a draw after typical recovery. Athletes should accurately report recent exercise rather than trying to interpret the pattern alone.

GGT and the broader pattern

Gamma-glutamyl transferase (GGT), alkaline phosphatase, bilirubin, and other findings may provide additional information about hepatobiliary context. In the weightlifting study above, GGT, alkaline phosphatase, and bilirubin stayed within their reference ranges while AST and ALT rose.

That pattern cannot be used as a home diagnostic rule. Normal GGT does not prove that the liver is healthy, and elevated GGT does not identify one specific cause. Symptoms, magnitude, persistence, and clinical history remain important.

When follow-up matters

Persistent, marked, or otherwise unexpected abnormalities warrant healthcare-provider review. So do symptoms such as jaundice, severe abdominal pain, confusion, persistent vomiting, unusual itching, or very dark urine. Urgent symptoms should be assessed promptly.

A healthcare professional may review training timing, alcohol, medications, supplements, viral risk, metabolic health, and prior results; repeat testing or additional evaluation may be appropriate. Do not stop prescribed medication or assume a supplement is safe based on an article.

Making the next test easier to interpret

If repeat testing is recommended, ask the ordering professional whether training should be modified beforehand and for how long. Keep other preparation conditions consistent where practical, follow fasting instructions, and document recent workouts, illness, and supplements.
