Cystatin C vs. Creatinine: A Kidney Marker That Isn't Skewed by Muscle
Creatinine-based eGFR assumes average muscle mass. Learn what cystatin C measures, why guidelines use it as a confirmatory test, and when it adds information for muscular athletes.
If you carry a lot of muscle, you may have had the experience of a creatinine result flagged high and an eGFR flagged low, with no symptoms and no obvious explanation. Cystatin C is the marker most often used to add clarity in that situation.
It is not a better test in every case, and it does not overrule a creatinine result. It is a second, independent estimate that happens to be built on different assumptions — which is exactly what makes it useful when the first estimate’s assumptions do not fit the person.
The limitation cystatin C is addressing
Creatinine is a waste product of normal muscle metabolism, filtered out by the kidneys. Serum creatinine works as an indirect kidney marker because filtration removes it — but the amount produced depends heavily on how much skeletal muscle a person has.
An eGFR equation converts creatinine into an estimated filtration rate using demographic variables. Those equations are built around population-average creatinine production. Someone producing substantially more creatinine than the equation assumes can be handed a lower eGFR than their kidneys actually warrant, and someone with very low muscle mass can be handed a falsely reassuring one. NIDDK notes explicitly that creatinine-based estimates are less reliable at extremes of muscle mass or body size.

What cystatin C is
Cystatin C is a small protein produced at a fairly steady rate by essentially all nucleated cells in the body. It is filtered by the kidneys and then reabsorbed and broken down, so its blood concentration reflects filtration.
The relevant property is what it does not depend on: production is not primarily driven by skeletal muscle mass. That makes a cystatin C-based estimate largely independent of the variable that distorts creatinine in muscular people.
How the two are used together
Cystatin C is generally not a replacement for creatinine. In current kidney guidance it appears as a confirmatory test — a way to check a creatinine-based eGFR when that estimate is thought to be inaccurate, or when a decision depends on getting the number right.
There is also a combined equation that uses both markers together. Because the two are affected by different non-kidney factors, a combined estimate is generally more accurate than either alone, and KDIGO guidance points toward using both when an accurate estimate matters. A clinician deciding between these approaches will consider why the estimate is being made and what will be done with it.
The pattern most people are hoping for is a creatinine-based eGFR that looks low and a cystatin C-based estimate that looks normal — a picture consistent with high creatinine production rather than reduced filtration. It is a plausible explanation, not a guaranteed one, and confirming it is a clinical judgement.
Cystatin C has its own confounders
Swapping one marker for another does not remove the need for interpretation. Cystatin C can be affected by factors unrelated to kidney filtration, including thyroid dysfunction, corticosteroid use, systemic inflammation, obesity, and smoking. Some of these are directionally opposite to the creatinine confounders, which is part of why combining the two is often more informative than choosing between them.
In other words: cystatin C is not the “true” kidney number. It is a second estimate with a different set of blind spots.
What neither test tells you
Both are estimates of filtration. Neither describes kidney structure, neither detects protein leaking into urine, and neither replaces the rest of a kidney assessment. A fuller picture typically includes urinalysis or a urine albumin-to-creatinine ratio, blood pressure, electrolytes, a medication and supplement review, symptoms, and change over time — often with imaging when clinically indicated.
Kidney conditions do occur in athletes. Marked swelling, reduced urine output, shortness of breath, severe weakness, or dark urine can require prompt medical attention regardless of how a filtration estimate reads.
When ordering cystatin C makes sense
It tends to be worth discussing when:
- a creatinine-based eGFR is unexpectedly low in someone with unusually high or low muscle mass;
- creatinine results have been inconsistent between draws;
- a medication or treatment decision depends on an accurate filtration estimate; or
- a clinician has specifically asked for a confirmatory estimate.
It is a poor choice as a standalone first test. Without a creatinine value from the same period, there is nothing to compare it against, and comparison is most of its value. Practically, that usually means ordering it alongside a comprehensive metabolic panel, which already contains creatinine and eGFR.
