Total vs. Free Testosterone: What a Testosterone Blood Test Actually Measures

Total, free, SHBG, and the sensitive assay all describe different things. Learn what each measurement represents and why the method and timing of the draw change the number.

“Get your testosterone checked” sounds like one test. It is closer to three or four decisions: total or free, which assay, what time of day, and what else is measured alongside it. Those choices change the number that comes back and how much it can be trusted.

This is an explanation of what each measurement represents. It is not guidance on treatment, and no value here should be read as a target. Testosterone results are interpreted against symptoms, history, and repeat testing by a healthcare professional.

What total testosterone measures

Total testosterone is the entire amount circulating in the blood, regardless of what it is attached to. Most of it is not floating freely — the majority is bound to sex hormone binding globulin (SHBG), and a further portion is loosely bound to albumin. Only a small fraction circulates unbound.

Total testosterone is the standard first measurement in most clinical guidance. It is well standardised, widely available, and for many people it answers the question on its own.

What free testosterone measures

Free testosterone is the fraction not bound to carrier proteins. The reasoning behind measuring it is that bound hormone is less immediately available to tissues, so the free fraction may better reflect biological activity in some circumstances.

The complication is measurement. Free testosterone can be measured directly by equilibrium dialysis, or calculated from total testosterone together with SHBG and albumin. Direct analog immunoassays for free testosterone have known accuracy limitations, and different laboratories use different approaches — which means free testosterone values are less comparable between labs than total values are.

This is why free testosterone is usually treated as supporting context rather than a standalone answer.

Why SHBG belongs in the conversation

SHBG determines how much of the total is bound. When SHBG is high, a normal total testosterone can correspond to a lower free fraction; when SHBG is low, the reverse can be true. The Endocrine Society’s guidance specifically notes that free testosterone is worth considering in people whose SHBG may be altered.

SHBG itself is not fixed. It can shift with thyroid status, liver health, insulin resistance and obesity, age, certain medications, and some illnesses. That is why a total-plus-SHBG combination often explains a result that total alone makes look contradictory.

Standard assay versus the sensitive assay

Not all testosterone measurements use the same method. A standard immunoassay is adequate across much of the adult male range. A sensitive assay — typically liquid chromatography with mass spectrometry (LC/MS) — resolves lower concentrations more precisely.

That precision matters at the low end of the range: for men with suspected low testosterone, for women, for adolescents, and for anyone whose value sits near a decision point. Laboratories and guideline bodies generally recommend mass-spectrometry methods when accuracy at low concentrations is required.

Practically, this means the assay is part of the result. Comparing a sensitive value to an older standard-assay value from a different lab introduces a difference that has nothing to do with your physiology.

Timing, and why one draw is rarely enough

Testosterone follows a daily rhythm, and in younger men it is typically highest in the morning. Reference ranges are built around morning draws, which is why guidance generally calls for a morning sample — commonly before mid-morning — for a comparable result.

There is also substantial day-to-day biological variation. A single value can land below a reference range and then sit comfortably inside it on a repeat draw. For this reason, clinical guidance consistently recommends confirming an unexpected low result with a second morning measurement rather than acting on one number.

Acute illness, poor sleep, recent hard training, and significant calorie restriction can all move a result temporarily. Recording the conditions of the draw makes a repeat far more informative.

What is often measured alongside it

Depending on the question, a clinician may look at testosterone together with:

  • SHBG, for the binding context described above;
  • LH and FSH, which can help distinguish a testicular cause from a pituitary or hypothalamic one;
  • Estradiol, measured with a sensitive assay in men where low-range resolution is needed;
  • Prolactin, which is relevant to certain pituitary causes;
  • A CBC, since hematocrit is monitored in some testosterone-related care; and
  • PSA, where prostate screening is part of the discussion.

What a testosterone result cannot tell you

A number does not, by itself, diagnose anything. Guidance describes hypogonadism as a combination of consistently low measurements and consistent signs or symptoms — not a lab value alone. Equally, a value inside a reference range does not rule out a problem that has another explanation entirely, including thyroid dysfunction, sleep apnea, medication effects, or an energy deficit.

Reference ranges also vary between laboratories and populations. A result near the edge of a range is a reason for a conversation and often a repeat, not a reason to conclude anything.

If you are ordering this test on your own

A few things make the result more useful:

  • Book a morning draw, and book future draws at a similar time.
  • Note recent illness, sleep, training load, supplements, and all medications.
  • Keep the report itself, including the lab name and the assay used, so future results are comparable.
  • Take an unexpected result — high or low — to a qualified healthcare professional rather than interpreting it against numbers from a forum.
