Bloodwork During Bodybuilding Contest Prep: What Athletes Should Understand
Contest preparation can change hydration, metabolism, recovery, and hormones. Learn how to approach bloodwork as context—not a standalone verdict.
Bodybuilding contest preparation commonly combines prolonged energy restriction, high training demands, and progressively lower body fat. Research in natural competitors shows that this process can coincide with changes in cardiovascular measures, resting physiology, mood, strength, and hormones. Responses vary, and the available studies are often small.
Bloodwork can provide objective context during a demanding phase. It does not certify that a preparation strategy is safe, and a normal-looking panel does not override concerning symptoms.
Why collection conditions matter during prep
Contest prep can make conditions less stable from one draw to the next. Food intake, carbohydrate availability, training load, sleep, hydration, and the point in the preparation timeline may all differ.
Document the phase, recent training, illness, supplements, medications, and whether collection instructions were followed. Comparing results gathered under reasonably similar conditions is generally more informative than treating every difference as a new physiological event.
This article does not provide dehydration, diuretic, drug, hormone, or peak-week instructions. Those practices can carry serious risks and require qualified medical oversight.
Hydration and fluid balance
Changes in fluid intake and losses can affect the concentration of blood components and influence electrolytes, creatinine, hematocrit, and other values. A concentrated sample can make some measurements appear higher without identifying the underlying reason.
Symptoms of significant dehydration or electrolyte disturbance—including confusion, fainting, chest symptoms, severe weakness, or an irregular heartbeat—can require urgent care. Do not manipulate hydration to obtain a preferred laboratory result.
Cardiovascular context
A lipid panel, ApoB when appropriate, blood pressure, resting heart rate, symptoms, and family history provide different pieces of cardiovascular information. Extreme leanness or high fitness does not guarantee low cardiovascular risk.

Metabolic markers
Fasting glucose, hemoglobin A1c, electrolytes, and other CMP measurements may be considered depending on the clinical question. Energy restriction and changing intake can influence physiology, while acute illness and collection conditions can complicate a snapshot.
A result should not be used to justify escalating restriction. New symptoms or significant abnormalities deserve healthcare-provider review.
CBC and oxygen-carrying cells
A CBC reports red cells, hemoglobin, hematocrit, white cells, and platelets. Fluid shifts can affect concentration, but they are only one possible influence. Persistent or substantial changes should not be dismissed as “just prep.”
Fatigue, dizziness, shortness of breath, infection symptoms, unusual bleeding, or clotting concerns need appropriate assessment rather than self-interpretation.
Kidney- and liver-related measurements
Creatinine-based eGFR can be influenced by muscle mass and other non-kidney factors, while hydration and rapidly changing creatinine can complicate interpretation. Creatinine and eGFR in muscular athletes covers these limitations without assuming an abnormal result is benign.
AST and ALT may rise after strenuous training in some athletes. Because liver and non-liver conditions can also elevate them, exercise is context—not an all-clear. Hormone and thyroid-related context
Studies of natural bodybuilding preparation have reported reductions in testosterone and other endocrine changes during prolonged low energy availability. Individual responses differ, and a laboratory value does not establish the cause of a symptom.
TSH and thyroid hormones may be considered when clinically relevant. Sleep, illness, energy availability, medications, and timing can influence endocrine measurements. Do not use results to self-prescribe hormones or change medication.
Recovery and inflammation
Training performance, soreness, sleep, mood, heart rate, and symptoms can add context that a lab panel cannot. Nonspecific markers such as hs-CRP may change with inflammation but do not identify a source. Creatine kinase may reflect muscle stress, yet magnitude and symptoms matter.
Severe muscle pain or weakness with dark urine after training may signal a medical emergency and should be evaluated promptly.
Compare trends without ignoring the present
A pre-prep baseline can make later changes easier to understand. Mid-prep and recovery measurements may be useful when ordered for a clear reason, but there is no universal schedule or test list for every competitor.
